Self-Check Health Screening

To ensure the safety of everyone on campus, we are following the California Department of Health COVID-19 guidelines requiring that everyone is screened for COVID-19 symptoms and exposure before entering Rocky Peak’s campus.

Self-Check Health Assessment Questions:

  1. Have you or any member of your household experienced any cold or flu-like symptoms such as: cough, shortness of breath, difficulty breathing, fever or chills, in the past 24 hours?
  2. Have you or any member of your household had a fever of 100.0F or greater in the past 24 hours?
  3. Have you or any member of your household had close contact with someone who tested positive for COVID-19 in the last 14 days?
If you answered yes to any of these questions, for everyone’s safety, we ask that you refrain from attending a Rocky Peak service or event.

COVID-19 Risk Assessment

An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, adults 65 and older and people with underlying medical conditions are especially vulnerable. By entering Rocky Peak’s campus, you voluntarily assume all risks related to exposure to COVID-19. For CDC Guidelines, please see www.cdc.gov/coronavirus
